1. The dog, who was homeless, had fleas. INCORRECT - this includes a subordinate clause
2. The man with the book, The Scarlett letter, used it to beat people with. INCORRECT (and pet peeve) - this is an appositive, not an appositive phrase.
3. The dinosaur, who was in fact not extinct, attacked the small town. INCORRECT - see #1
4. David, the English scholar, wrote essays with a passion like no other. CORRECT
5. Sally, my niece, loves Disney movies. CORRECT

1 and 3 are correct, but 2 contains an appositive (three words are evident, but it is all - collectively - one thing)
